Job Summary:
The Population Health Coordinator (PHC) provides the infrastructure that allows us to optimally and proactively manage our populations of patients with chronic conditions (e.g., diabetes, hypertension, asthma) and those in need of preventative screenings. The PHC will support practices with short-term gap closure focused specifically on hypertension.
Duties:
- Work collaboratively with practice teams to implement identified strategies to ensure that patients receive the condition specific services they need
- Manage patient registries and related data and provide members of health care teams in designated practices with patient data
- Conducts timely outreach tailored to meet each patient’s condition specific needs
- Learn and understand Primary Care practice workflows with respect to optimal and coordinated health care for target patient populations
- Provide regular feedback on the status of on-going work with the practice teams and attends practice and central team meetings as directed
Experience/Skills:
- Bachelor’s degree
- Strong communication, customer service, and organizational skills and a willingness to learn quickly
- Experience working in a healthcare setting with providers
- Experience working with an electronic medical record
- Experience with Epic is preferred
- Experience with PC / Microsoft applications including Outlook, Word, Excel, and PowerPoint
